- Make a new document. Keep the background as white.

Make sure the foreground color is black.
Pick a brush and start brushing.
I came up with this:

Make a new layer and change the foreground color to #333333.
Get a new brush and start brushing.
I came up with this (just more foggier):

Make a new layer and change the foreground color to #666666.
Get a new brush and start brushing.
I came up with this (again, more fog):

Make a new layer and change the foreground color to #999999.
Get a new brush and start brushing.
I came up with this (you think that's enough fog?):

Make a new layer and change the foreground color to #FFFFFF.
Get a new brush and start brushing.
I came up with this (okay :D):

Start messing around with the blending modes.
I made the blending mode for layer 2 as: Multiply.
And for layer 4 as: Overlay.

Make a Color balance Adjustment Layer.
- Modify all: Shadows, Midtones, and Hightlights to your liking.

Repeat steps 8 and 9.
But for 9, try to focus on one or two points.
I try to focus more on highlights and shadows on the 2nd time around.

Mess with the blending modes.
For the first Color Balance Adjustment Layer i used: Multiply.
For the second one i used: Soft light.

Add a Pattern Adjustment Layer.

Choose black scanlines, 2 pixels apart. (If you dont have any black scanlines, refer to my simple tech layout tutorial).
- Change the Blending mode to Soft Light, and the Opacity to 30%.

Add a Hue/Saturation Adjustment Layer.

Choose a color similar to what you have so far, make sure "colorize" is checked.
Apply it, then change the Blending mode to Multiply.
- Now make a selection on that layer diagonally across, I selected upper portion.
- Right-click on selection, and choose: Feather: 20px, and press delete.

Now, choose a nice matching render.
Drag it in.
Resize it till it looks good to you.
Hold Shift when resizing to keep everything in proportion.
Shift+Alt when resizing may also be useful.
- Put the render under one of the Adjustment layers till it matches the background.

Make a selection around the render, where you think might add a nice blending with the background.
I chose these areas:

Then feather it: 20px.

Right-click: Select inverse, and apply a layer mask.
If need be, lower the opacity for the render.
I lowered it down to 80%.
- Make a new layer.
Select all (Ctrl+A).
Edit > Stroke: 1px Black.

Use the Magic Wand tool to make a selection inside the border.
Then, Select > Modify > Contract: 1px.
Then, Edit > Stroke: 1px Black.
- Add some text, apply some Blending Options, and we're done.
